• Regular viral load/CD4 monitoring
• Long-acting injectable HIV treatment (if eligible)
• Management of co-existing conditions like diabetes or hepatitis
• Access to labs, pharmacy services & refills
• Case management, coordination, and patient advocacy
• Referrals for dental, mental health, housing, and legal services
• Education and support around U=U (Undetectable = Untransmittable)
